Family Support Services Program
Imagine!'s Family Support Services Program is here to assist families who provide at-home care to family members with intellectual and developmental delays or disabilities. Our services are designed to ease the financial and emotional strain of having a family member with a disability in the home. Additionally, we provide grants to eligible families who need extra funds to help pay for items and services for their family member in need.

What
We provide access to Family Support Navigators and limited financial support. Families are offered grants based on available funds in a given fiscal year. Family support grants can help pay for the extraordinary costs of caring for a child or dependent with intellectual and developmental delays or disabilities. Grants are distributed at designated times throughout our fiscal year and are based on available funds. A list of approved expenses can be found by clicking the link below.

Who
We have services that are available to families with an individual with an intellectual or developmental delay or disability who: lives at home and resides in Boulder or Broomfield counties.
When
Our fiscal year runs July 1 through June 30th of the following year. Grant applications are automatically sent to eligible families at designated times throughout our fiscal year.
How
Once the family support team receives a completed Most In Need assessment, a grant application will be mailed during one of three designated grant runs throughout the year.

What is a Family Support Navigator?
-We assist in applying for financial assistance
-We provide information about community resources
-We assist in accessing referrals.
Starting the FSSP Eligibility Process
Step 1: Please reach out to the Imagine! Intake Department at intake@imaginecolorado.org to complete the eligibility requirements.
Step 2: Once eligibility for Family Support has been confirmed, a member of the Imagine! FSSP team will send a welcome letter with a Most In Need (MIN) assessment attached. Once we have received your MIN assessment back, your family will become eligible to receive a grant.
Step 3: An Imagine! Family Support Navigator will reach out to you with next steps in regards to the grant application and funding process.
