Case Management Redesign History
A short timeline of CMRD, including a glimpse at the future:
2014
Federal legislation enacted requires ….”that assessment and coordination of services are separate from the delivery of services. Providers of Home and Community Based Services (HCBS) for the individual must not provide case management activities or develop the service plan. Conflict occurs if they are the provider, are employed by the provider, or have an interest in the provider.”
2017
Colorado HB 17-1343 was passed to implement the Centers for Medicare/Medicaid Services (CMS) rule in Colorado. Under this legislation, Community Centered Boards (CCB) must choose between one of the three following options:
- Offer case management only
- Offer direct services only
- Provide both but not to the same person (enrolled in HCBS services)
February 2020
Health Care Policy and Financing (HCPF) announced their plans to implement Case Management Redesign (CMRD) in Colorado to create a better system of supports for people that is also compliant with Conflict Free Case Management. Former plans no longer valid.
March 2021
HB 21-1187, “Concerning the Implementation of Case Management Redesign to Ensure Conflict Free Case Management for Members Eligible for Long Term Services and Supports Under the Medicaid Program” outlines Case Management Redesign:
- Establishment of a redesigned case management system no later than July 1, 2024
- By December 31, 2021, HCPF will work with stakeholders to create a timeline for implementation
- By December 31, 2022, HCPF shall issue a competitive solicitation process in order to select case management agencies
Throughout 2021
HCPF hosts regular stakeholder meetings with CCB and Single Entry Point (SEP) leadership.
Fall 2021
HCPF announces new catchment areas. Ours are the counties of Boulder (existing), Broomfield (existing), and Gilpin (new).
January 2022
HCPF hires Health Management Associates (HMA) to partner in implementation of CMRD. Their charge is to conduct needs assessments of Case Management Agency (CMA) readiness and create technical assistance plans. The state is spending $4.3M of ARPA funding on this.
March 2022
HMA asks CCBs to complete a survey on readiness for CMRD.
The Timeline –Now through July 2024
July 2022 through June 2024
HMA is contracted to provide ongoing technical assistance to CMAs to implement their transition plans, whether it be as an awardee of the Request for Proposal (RFP) or to close out.
