Timeline of Case Management Redesign

What we have accomplished so far:

  • NDA signed by executive leaders, Board Members, and other key staff of both organizations (September 2022)
  • Intent to Combine Case Management Operations signed by executive leaders, approved by Boards of Directors (November 2022)
  • Creation of a Joint CMA Subcommittee by both organizations' Board of Directors, assigning current Board members (total of 6) to serve in an advisory capacity to the CEO of Imagine! and the E.D. of ACMI during the development of the response to the RFP proposal and transition to the intended newly combined CMA following the award results of the submission (January 2023)
  • Completion of a culture survey conducted for all case management staff of both ACMI and Imagine! to capture important information each culture considered essential for current staff (January 2023)
  • The executive leaders of Imagine! and ACMI establishes a regular meeting schedule to continue to update and meet goals highlighted on the combined project plan with a timeline. (January 2023 through Present)
  • Submission of the RFP created collaboratively by the E.D. of ACMI and the CEO of Imagine! with assistance from a handful of key staff from both organizations (February 2023)
  • Approval by the Imagine! Board of Directors of a strategic work plan for the continuation of Imagine! as the PASA, reaffirming that direct services will continue but as a standalone agency for the future and assignment of Board members to form a PASA Future subcommittee which serves as an advisory body to the CEO and executive leadership as the development and execution of the plan to create a standalone PASA organization proceeds through 2023 (March 2023)
  • Introduction and initial transition to TriNet PEO.(December 2023)
  • Developed process for selecting Board members for PASA and CMA.(January 2024)
  • Process and job description developed for selecting executive leadership for the PASA.(January 2024)
  • Posted releases for CEO position for Imagine! PASA. (February 2024)
  • Completed review of potential new systems (e.g., HR, IT, Accounting). (February 2024)
  • Assessed physical space requirements. (February 2024)

March 2024

  • Continue communications for internal and external stakeholders.

  • Begin implementation of new systems if relevant (e.g., HR, IT, Accounting).

  • Develop organizational and financial structures for PASA and CMA.

  • Continue legal work for each separate organization.

April 2024

  • Complete final phase of legal work to separate entities, including registration of new CMA with State of Colorado and changes to definition of Imagine! as direct services only.

  • Complete announcements of key executive and leadership roles for PASA and CMA.
  • Early Intervention completes provider enrollment verification and prepares for move to A&I Avenues.
  • CMA completes Targeted Case Management enrollment verification.
  • Celebrate Imagine! and the future at Spring Party for Imagine! employees plus one guest.

May 2024

  • CMA processes all client requests for exceptions to service area boundaries.
  • Build out of various new/enhanced systems for CMA (e.g., IT, Accounting, Electronic Health Record).
  • Complete enhancements of systems for PASA.
  • A&I Avenues website development continues with content review and refinements.
  • Confirm staffing and organizational charts, policies and procedures for PASA and CMA.

June 2024

  • Complete legal, financial, organizational and systems work to "go live" with Imagine! PASA and CMA on July 1.
  • Complete physical moves.
