Community Partners Program
Promoting Wellbeing and Independence
The Community Partners Program is designed to provide comprehensive support for residents, particularly those with intellectual and developmental disabilities (I/DD). Our goal is to help individuals build essential life skills, connect with meaningful services, and foster independence. Through peer mentorship, skill-building sessions, and personalized guidance, we offer practical tools to help residents thrive in their daily lives.
We believe that everyone deserves the opportunity to live with independence and dignity. However, navigating the complex system of care and services can be overwhelming, especially for individuals with I/DD. That’s why we are committed to:
- Supporting holistic wellbeing by addressing both physical and mental health needs.
- Encouraging self-advocacy to help residents make informed decisions and take control of their lives.
- Building sustainable support networks to ensure residents have ongoing access to the resources they need for long-term success.
By working closely with residents and community partners, we strive to create a more inclusive, supportive environment where everyone can achieve their full potential.
Support for Residents

Peer Mentorship
Our program connects residents with mentors who have lived experience, creating meaningful peer-to-peer relationships. Through shared experiences, mentors offer emotional support, practical advice, and guidance to help residents:
- Build self-confidence and independence.
- Set personal goals and track progress.
- Navigate challenges in daily life, work, and relationships.

Skill Building for Daily Life and Beyond
We believe that developing new skills is essential for long-term success. Our team works closely with residents to identify areas for growth and provides individualized coaching in areas such as:
- Budgeting and personal finance management.
- Household management and organization.
- Job readiness, including resume building and interview preparation.
- Social skills to foster community engagement and meaningful connections.

Navigation and Advocacy for Essential Services
Understanding and accessing the right services can be a daunting task. Our experts are here to help residents cut through the complexity by:
- Providing personalized guidance on available programs, including healthcare, housing, and employment services.
- Assisting with paperwork, applications, and service enrollment.
- Offering advocacy to ensure residents’ voices are heard and needs are met.
