Our newest addition to Imagine!'s Mental Health Services offers a comprehensive evaluation that looks at every part of your mental and behavioral well-being, empowering you to find the right services that fully support your unique needs.
Our Unique Evaluation Process
Our evaluation process involves a team of skilled professionals, including a Psychiatrist, Psychologist, Board Certified Behavior Analyst (BCBA), and Licensed Clinical Social Worker (LCSW). Together, they bring their expertise to create a thorough understanding of your needs and provide recommendations tailored specifically for you.
Getting Started
The first step towards joining our program is to complete our accessible & HIPAA-compliant referral form, available by clicking on the following link: Mental Health Clinic Form. This form will help us assess if you are a good candidate for our evaluation process. Please make sure to include every detail important to your health so that we can better complete our assessment.

Program Capacity
Because our services are very detailed, we can only help two people each month. This lets us give the best care and make sure each person gets the attention and resources needed for a full evaluation. We keep an eye on our waitlist and hope to help as many people as we can in our community.
Community Support
We are proud to offer this service through our partnership with Boulder County and the utilization of Mill Levy tax dollars. This funding allows us to provide these essential services and support the well-being of our community members.
Time Commitment
This full evaluation will take 5-10 appointments, lasting 20-40 hours over 30-45 days. Sessions can happen at our office, your home, or other places in the community. We know this is a big commitment, so please think about your schedule before applying. We’re happy to talk with you more about it if you have any questions.
