Family Support Services Program (FSSP)
Imagine!'s Family Support Services Program is here to assist families who provide at-home care to family members with intellectual and developmental delays or disabilities. Our services are designed to ease the financial and emotional strain of having a family member with a disability in the home. Additionally, we provide grants to eligible families who need extra funds to help pay for items and services for their family member in need.

Developmental Disabilities Waiver (DD)
The Developmental Disabilities Waiver (DD) provides a safe home and community activities for people who need assistance from others 24 hours a day. Residential settings can range from living with a paid caregiver, family caregiver, rotating staff, or private apartment with caregivers checking in. The right residential setting is chosen based on the person’s preference and medical or behavioral needs. There is a waitlist for this waiver.

Supporting Living Services Waiver(SLS)
The Supported Living Services Waiver (SLS) is for people who are very independent or people who have lots of natural or community support. The person on this waiver lives with family or in their own home and wants to add services to their Medicaid so they can continue to do so. There is no waitlist for this waiver.
