Navigating the Intake Road Map

When you or a loved one requires services for intellectual or developmental disabilities, applying for care can sometimes be difficult and frustrating. At Imagine!, we want to make that process as painless as possible so that you can get care as soon as possible. In this post, we will breakdown the process step by step as well as include an instructional video that walks you through intake. If you should have any questions at any point, feel free to contact the Imagine! Intake Navigator at intake@imagincolorado.org or (720) 274-5452.
Step One: Call or email the Imagine! Intake Navigator to discuss the criteria for Intellectual/Developmental Disabilities - intake@imagincolorado.org or (720) 274-5452.
Step Two: Wait for the Imagine! Intake Navigator to send you the application. This will happen within 2 business days. If you don't hear back, feel free to call or email the Intake Navigator with questions.
Step Three: Complete the Intake Application. For further instruction on how you can do this, head on over to this webpage: Completing the Online Application.
Step Four: The Imagine! Intake Navigator will send you supporting documents to fill out too. Make sure you turn those in within 90 days.
Step Five: Wait for the Imagine! Intake Case Manager to reach out to you. While you are waiting, they will be completing an I/DD determination. This should take no more than 30 days.
Step Six: If the Imagine! Intake Case Manager determines that you have an I/DD, they will discuss waivers and programs that are available to you. If they determine that you do not have an I/DD, they will offer resources and referrals to programs outside of Imagine!.
And remember, you can always contact the Imagine! Intake Navigator if you get stuck at any point during the process. They are always happy to help!
