This week we are joined by Kristen Lundy as she shares an update on Imagine! Case Management.
As we are nearing the end of the fiscal year, Case Management finds itself in the same shuffle that we experience every year at this time. We are diligently working to complete end of the fiscal year reporting for the department of Health Care Policy and Financing (HCPF) as well as complete quarterly monitoring visits for the over 1200 individuals that we serve. In addition, we are also in the process of enrolling 52 individuals (46 from the passage of the long bill and 6 from the regularly occurring churn) into the DD waiver with an authorization date of 7/1. To support with these enrollments and ensuring that families have all of the necessary information to make an informed decision, Imagine! Case management has been hosting informational sessions. Our first session was 6/22 and went incredibly well with lots of attendees who were actively engaged in the content. I have been fortunate to have a team of proactive leaders in our department who have put in a lot of hours to prepare for these sessions and ensure that families leave with all of their questions answered.
