Care Coordination Appreciation Week - Case Management

This week we are celebrating Care Coordination Appreciation Week, where we take time to show our gratitude for the fantastic work that the Imagine! Care Coordination team has done.

Today we are highlighting Imagine! Case Management, which acts as the gateway to services for people with disabilities in Boulder and Broomfield counties. The team includes Case Managers for Children, Case Managers for Adults, and Intake Case Managers.
